Your First Orthodontic Visit

We strive to make sure your visits with us will be pleasant and informative. Your initial appointment will consist of an examination and a discussion of potential treatment options with an estimate of the total cost for recommended treatment. The purpose of this initial visit is to allow us to determine the specific needs of each patient. We will be identifying orthodontic issues, the nature of the problem, the timing of treatment, and potential solutions.

In addition, we will review estimated costs of proposed treatment. When you leave our office, we want to be sure that all of your questions are answered and your dentist's concerns have been addressed. Then you will know exactly what next steps are needed in order to initiate treatment.

Please assist us by providing the following information at the time of your first visit:

Completed Patient Registration Forms and signed Patient Privacy (HIPAA) agreement.
If you have dental insurance, please bring the necessary information such as your insurance card. This will save time and allow us to help answer quesions about coverage for proposed treatment.

Emergency Situations

We try our best to stay on schedule to minimize your waiting and will schedule your appointment as promptly as possible. If you have pain or an emergency situation, every attempt will be made to see you that day.

Due to the fact Dr. Horwitz provides many types of orthodontic services, various circumstances may lengthen the time allocated for a procedure. Emergency cases can also arise and cause delays. We appreciate your understanding and patience.

Financial Policy

Financial considerations should not be an obstacle to a beautiful smile. Our practice offers a wide range of payment options including financing with no initial payment and a monthly installment plan. Many orthodontic problems require only limited treatment. The cost of treatment will vary, depending on the severity of the problem. Our treatment coordinator will work out a plan suited to your individual needs.
